AHCCCS is committed to assisting federal, tribal, and local law enforcement agencies in the search for missing persons. To that end, AHCCCS is able to provide information about enrolled members, when permissible by law (45 CFR 164.512(f)(2)), to assist law enforcement agencies in locating missing persons.
The requesting law enforcement agency must complete the Request for Data form with as many details as possible and attest that the data will not be used against the victim and is in the best interests of the member.
This process is available only to law enforcement agencies. Submissions by entities other than law enforcement will be returned.
