AHCCCS is committed to assisting federal, tribal, and local law enforcement agencies in the search for missing persons. AHCCCS is able to provide information about enrolled members, when permissible by law (45 CFR 164.512(f)(2)), to assist law enforcement agencies in locating missing persons. The requesting law enforcement agency must:

  1. Download the fillable Request for Data Form
  2. Complete all form fields with as much detail as possible and attest that the data will not be used against the victim and is in the best interest of the member.
  3. Sign with a valid digital/electronic signature.
  4. Email form to missingpersons@azahcccs.gov.

AHCCCS will vet the data request and return an acknowledgement of receipt within 5 business days. This process is available only to law enforcement agencies; other entities should call 911 as submissions will be returned. Share this Law Enforcement Data Request flier with city, county, state, tribal, and federal law enforcement agencies.

Call your health plan first! If you cannot access health care services, or need help with your AHCCCS services, call your health plan. Visit Health Plan’s contact for questions, information to change health plans, or for additional information.

  • Escalate Issue to Clinical Resolution Unit: If you have reported your issue to your health plan and need additional help, contact 602.364.4558 (toll-free: 1.800.867.5808).
