TI Year 4, 5 & 6 Milestone Performance Measures
This applies to TI 1.0
TI Program Year 4, 5, & 6 milestone incentive payments were based on performance measures in contrast to the Core Components used in TI 1.0 Years 2 and 3.
Participants will receive incentive payments based on their performance on the measures as calculated via claims with dates of service in each program year:
- Year 4: October 1, 2019 through September 30, 2020
- Year 5: October 1, 2020 through September 30, 2021
- Year 6: October 1, 2021 through September 30, 2022
As can be seen from the noted measure sets, the metrics align with various other applications and uses, including the CMS Core Set and Statewide Targeted Investments measures.
The measures' calculation and target methodology can be found at TIPQIC.org. AHCCCS provides ongoing engagement and support to TI participants’ efforts to meet the performance measure milestones, including peer learning, technical support, intermittent feedback on measure performance, and guidance on performance improvement through the end of the program.
Year 4, 5 & 6 Milestone Performance Measures
Participation in the QIC was a Year 4, 5 and 6 Milestone requirement. To meet the milestone, the organizations participating in the program (TIN level) had to attend the kickoff meeting and a majority of the virtual sessions. Please see the list here for the number of representatives who must attend from your organization.
The organization’s administrative and clinical QIC representatives (excepting one site participants as noted above) or their designees must attend four of the Year 6 Quality Improvement Collaborative virtual group meetings offered for the Area of Concentration.
Alternatively, organizations may complete the milestone by attending three QIC virtual meetings AND two Quality Improvement Workgroups.
YEAR 5 - YEAR 6 CORE COMPONENTS & IPAT SCORE: Participants needed to attest to Core Components 1 and 2 in addition to entering the IPAT scores to complete the attestation process. The IPAT Assessment Tool can be accessed here.

TI Years 2 & 3 Core Components & Milestones

Year 2 (October 1, 2017 - September 30, 2018) and Year 3 (October 1, 2018 - September 30, 2019) of TI 1.0 participants were asked to develop processes and procedures that aligned with TI 1.0 program initiatives for each participating area of concentration. After the clinics designed and implemented the procedures during Year 2, participants conducted audits to demonstrate that the new procedures were utilized at least 85% of the time.
